Access Wellbeing Support with Activities on Prescription

Activities on Prescription is Nuvo Wellbeing social prescribing programme initially funded by the National Lottery Fund to help boost wellbeing and promote positive behaviour change across County Durham and Hartlepool over 24 months. Referrals were granted access to a range of resources including:

  • Weekly Dance and Fitness Sessions 
  • Monthly Arts and Singing Events 
  • Resource Packs in the post
  • An online library of workshops 
  • Regular check-in calls from our friendly team

Through our referral pathway with local social prescribing teams, we engaged with 112 individuals living in County Durham and Hartlepool for 6 months or more to promote long-term behaviour change.

  • 97% of those who took part reported that they are now more physically active
  • 88% stated they felt they were more socially included in their local community.

    As our National Lottery Funding has come to an end, this service is now available for purchase by community health organisations and local authorities for their services users in all areas of the UK.
